To enter kindergarten, a child must be 5 years of age on or before October 1 of the enrollment school year.
The parent/guardian must bring a certified birth certificate to registration.
The parent/guardian is requested to provide the child’s social security card at registration.
In compliance with the KY State Board of Education and Public Health regulations, parent/guardian MUST (on or before the first day of school) have provided the school with the following forms:
KY Immunization Certificate showing all age appropriate vaccines including: Diptheria-Pertussis-Tetanus (DPT) and Polio Vaccine (IPV or OPV) last dose given after age 4, two doses of Measles-Mumps-Rubella vaccine (MMR) second MMR due at age 4, three doses of Hepatitis B vaccine (HBV), H influenza vaccine (Hib)-for preschoolers only. Chickenpox vaccine (Varicella) one dose or a note stating that child has had the disease.
School Medical Examination Form completed by family physician within 1 year before first time school enrollment.
An eye exam is required that has been performed by an Optometrist or Ophthalmologist.
A dental examination is strongly recommended before entering school.
Two (2) forms for Proof of Residence (renters/lease agreement), utility bill (within last 30 days) (Power, gas, electric, residential telephone, (cell not accepted) cable, sanitation or water)
